Types of Collaborations Present with Local Community Resources	Board of Social Services provides emergency housing. Hospital provides physicals. Local dentist provides emergency dental care. Career Center provides employment info and help and the Food Bank provides bulk food.
Aspect of Program that is Innovative or Unique	We will have an educational building next to the home which will allow us to provide outclient services seperate from the resident's home and eliminate transporation barriers for residents to come in for services. Our 3 apartments will be next to home also.
Program's Biggest Challenge	Follow up. Initial follow up is easy but after a period of time we lose contact with youth because they move around and don't keep us informed adn phone numbers we had for them are no longer in service. Youth seeing initial sucess and wanting to move on before completion of their treatment plan.
